Recruitment Steps

1. Fill out and sign the membership declaration. The form is available for download below. Please print it, complete it legibly, and sign.

2. Send a scan/photo of the signed declaration to the following e-mail: sekretarz@ptbn.pl.
   In the subject line, please write: “Membership Declaration – First Name Last Name”.
3. Wait for confirmation of receipt of the documents. Only after receiving confirmation, please proceed with the membership fee payment.

The membership fee can be paid through the “Pay the fee” tab, located in the About us menu. It is also possible to make a payment by traditional bank transfer – the bank details are provided in the declaration.
